[squid-users] Authentication failed

Dear members
I used ncsa_auth module for squid authentication...but
always i get authentication faild messege..:(
once i used a file containd <usernames>
<passwords>..it did not work...then i use the
/etc/passwd file,did not work....my squid conf is like
this for authentication

authentication_program /usr/lib/squid/ncsa_auth
/etc/passwd

please help me
