This looks like your Squid is talking to itself.
What is said in cache.log? Every message from Squid is logged in
cache.log, and some ("important" ones) are CC:d to syslog.
You haven't configured your Squid as a peer of itself have you? While
this is a very stupid thing to do, it can easily happen if you mainain
more than one Squid and accidentially install the config for another
server.
Any transparent proxying configured? There are many sources of errors
like this in transparent proxy setups, especially if you are also
peering with other proxies..
