Above means that some response headers were received which Squid
believed should have a body attached. No body was received before the
server closed the TCP connection.
The xxxxx are the requested URL before and after adaptation by
url_rewrite / ICAP /eCAP to assist with tracking the problem down via
access.log or other data.

The huge ctx levels are a sign that your particular version has a "leak"
in ctx objects used to track partially received requests. This was found
and fixed in 3.1.5 (http://bugs.squid-cache.org/show_bug.cgi?id=2945)
