In our case, we maintain the database and radius server. I believe the
squid server (or proxy) will need to inject the 'http request header'.
It would need to build the 'http request header' based on information it
pulls from a database. I see that it does some request header
manipulation already (mostly changing existing headers) and was hoping
someone had used them in a similar fashion that I'm hoping to do.

Sometimes one is so close to the details that it's hard to put them all
into words everyone will understand. I did attempt to do so. :) I do
appreciate the feedback and nudges in the right direction. Hopefully
with a few questions I can get all the information out there that is
needed.

IIRC, your issue was paying whoever maintains your
DB or radius server. It isn't hard to write a cold fusion
page or java server to modify headers based on
a DB lookup if that is all you need. I'm not sure on
best approach if you want to rewrite headers
in squid but I'm still not even sure which step is of
most concern- accessing a DB or header modification.
It may be that you haven't gotten an answer because
other skimmed your post with same level of
detail and re-wording would help :)

surf their site. The header is x-msisdn. The value should be the
subscriber's phone number (mdn). We have been unsuccessful at finding a
good/solid solution for retrieving the mdn and inserting an http header.
The mdn (in our case) is stored in a database, so the information is
easily available. However, we do not know how to make squid request the
mdn and inject the http request header.

stored in a flat text file (radius accounting detail log)
