Web browsers contain their own caches which pretty effectively obsolete
single-user proxy caches.
Its likely with a single user that you are getting HITs in the web browser
cache instead of the proxy cache.

First off do a manual double-check:
grep "_HIT" /var/log/squid/access.log
(if calamaris was right, there will be no entries returned)

Second thing is to double check your config file. If you still think its
right post it here so we can see.
