Something like
http://wiki.squid-cache.org/ConfigExamples/Reverse/MultipleWebservers,
except with a second layer of reverse proxies, right? Use the
MultipleWebservers config on the 1st tier, and
http://wiki.squid-cache.org/ConfigExamples/Reverse/BasicAccelerator on
each of the 2nd tier servers.
