I used to have the following config in squid.conf:
auth_param ntlm program /usr/local/squid/ntlm/libexec/fakeauth_auth / 
usr/local/squid/ntlm/etc/passwd
This would allow me to enter any creds and "authenticate" with the  
squid proxy.
Using 3.0.HEAD, I see the following when a client attempts to auth:

Any idea how I can setup a "bogus" NTLM authenticator?  I only want to  
test NTLM auth between the client and squid proxy.  I don't care that  
it goes to a real Windows domain and auths.
