No matter how you reorder on Squid the auth_param, it's allways
displayed the Basic one (even if you comment it!!!).

The login dialog has 2 fields. At certain point it is normal, that if
browser didn't acknowledge the NTLM option it won't prompt to you an
NTLM popup.
