I didn't in recent months on the SQUID list. Neither do
my users experience this problem.

- Is this problem 'tied' to a particular browser, does
it happen with other browsers too ?
- Check Squid's access.log for those pages/images , anything
suspicious in there.
- Squid version ?
- Os/platform/version ?
