Looks like your parent is either unreliable or overloaded and from time to
time refuses to accept new requests, and as you are using never_direct
there is no other option for your Squid than to fail the request when the
parent is not functioning.
If your Squid can go direct to the Internet then don't use never_diret. If
you use never_direct then Squid will not attempt to go direct if using the
parent fails.
