Configure a parent cache using a cache_host or cache_peer directive
(depending on what version of Squid you have) in your squid.conf file. See
the "squid.conf.default" file included with the Squid distribution for
details.

Do you mean an NCSA .htaccess password file? Run "configure" with
"--enable-auth=basic" and "--enable-auth=NCSA". Then see the "auth_param"
documentation in the squid.conf.default file.
