why is there a forwarding loop?

I have two Linux 2.4 box with squid 2.4STABLE6.
I try to make them both work as transparent proxy,
but failed. The scenario is like below:
Client --> trans proxy A --> trans proxy B --> Server
The access.log of box B shows that the connection
does reach it, but the corresponding entry is a little
bit strange. The Elapsed Time of the connection last only
a few milliseconds, the Log Tag/HTTP Code is "TCP_MISS/403",
but the Hierarchy Data/Hostname is "NONE/-".
Turn down squid of any one of the box, and use SNAT to route
packets through would make the transparent proxy work properly.
I already set http_access allow all AND always_direct allow all.
Do I still miss something? Or the transparent proxy is supposed
won't work like this?
