I've got squidNT-2_3-stable4 setup in httpd-accelerator mode,
squid is configured to cache jsp pages for 1 hour using refresh_pattern.
At the moment we've got a problem where squid occasionally returning pages
that are several days out of date, and will continue returning the outdate
page for the 1 hour period.
When this occurs there appears to be a number of TCP_SWAPFAIL_MISS:DIRECT
in the access log. I've include a section of the access log below.
I'll appreciate if someone could shed some light on what is going or how to
resolve it.
