Normally you tell to configure what authentication helpers you want to
have built. This will generate the needed Makefile's from their
Makefile.in and include them in a standard Squid build.
From a Squid-2.4 source tree:
./configure --enable-auth-modules="NCSA"
make
cd auth_modules/NCSA
cp ncsa_auth /usr/libexec/squid/
