When you hit refresh your browser typically forces a retrieval. If you
want to test for access, don't use refresh, instead browse elsewhere,
clear you browser cache, and then go back to your test site.

You could look at the offline mode in squid, this could be extended I
suspect to do what you want.

The first one is nearly a FAQ, due AFAICT to the assumption that
"refresh" will still get cached hits.
