NO! As I explained last time, this is NTLM's manual password entry
screen.

I cannot remember - Are you running squid-HEAD, the ntlm branch, or the
auth_rewrite branch?
No-one else has reported this to the list, and Kinkie and I, the folk
responsible for the current code aren't seeing it (or we'd have fixed
it).
Can you try a clean configure and bulid with CFLAGS='-g'
