This has nothing to do with where Squid looks for a URL - it simply means
that once a URL has been retrieved, if it matches the ACL it will not be
added to the cache. The result of this is that Squid will (usually) check
its cache for the URL and return it if found - this can be confusing, but
can only occur if an object is cached before you activate the 'no_cache'
directive.
