a) Try calamaris-2.27. Though if my suspicion is correct, I don't think
   that will help.

b) Have you checked those lines? I suspect that they contains whitespace,
   and thus your average log line does not have 10 columns, but 10+.

   If you disabled ident looksup with your squid, there is a change to
   cover 99+ % of URIs with whitespaces, but patching calamaris is not
   everybodys favourite way to spend time...

   Enabling "uri_whitespace encode" feature in Squid might work around
   this, or might not (How are URLs logged to access.log?). It might not
   be an option for toplevel caches, either.
