A number of possible causes:
1. You Squid user (the one configures as cache_effective_user in
squid.conf) is not allowed to run cachemgr. Check filesystem permissions
on dnsserver and the directories up to where dnsserver is installed.
2. dnsserver is broken in your installation. Try running it manually and
type a host name. If it fails to return the IP address of this host then
something is wrong here.
3. Squid does not know where the dnsserver binary is located. This can
occur if your Squid is compiled with one installation path and then
istalled in another location. Either reinstall Squid at the proper
location or set the location in squid.conf.
