I had the same problem using 1.2beta24 a few weeks ago. I solved it by
adding:
acl parent.host dst 1.2.3.4/32
miss_access allow parent_host
to fetch digests from parent.
But I don't understand why I have do to that. I thought miss-access controls
who can fetch misses from my cache and not if I can fetch misses from parent.
I think that should be configured in the parent's squid.conf.
Could you explain that mechanism to me, please ?

PS: As I noted above I use squid-1.2beta24, so it's not an 'old squid'
