Squid's removal policy: Need-based or periodic?

In general, is the removal policy of Squid periodic?

If I want to change it to need-based, other than removing
eventAdd(storeGetMaintainSwapSpace), which function should I change,
file_write of disk.c or storeSwapOutHandle in store.c?
