acl aclname acltype "file"

I want to include an acl file, but I'm not sure of the syntax.
acl aclname acltype "file" ...
   would this translate too
acl passwdfile src /etc/squid_passwd

Each time you change the squid.conf acl list, what command do you enter
to make the changes live? As kill USR1 pid or kill HUP pid doesn't make the changes live, only kill -9 and restarting the server does.
