And at this time, your Squid process is huge (it wasn't at the
beginning). And fork has to copy it, and you do not have enough space in
virtual memory. (I had the problem with the dnsservers.)

Increase virtual memory. Add swap space. Or try to decrease Squid's usage
of memory (this is harder, specially if you want it to be fast). What is
your Squid's size ("ps aux|grep squid") and your swap use (highly
system-dependent, "swapon -s" on OSF1, "free" on Linux)?
