beta16 out, plus patch

Hi Folks,

Version beta16 is out and so far seems much better than the
previous four versions. However I have found one problem
already. If your cache gives a "fatal dump" with these messages:

    ipcache.c:196: ipcache_release: Someone called on a PENDING entry
    FATAL: ipcache_release: expected i == result!

then you need to get this patch:
    http://www.nlanr.net/Squid/1.0.beta16/ipcache.c.patch
