send_hit and store_miss are now committed to trunk (r13190). Besides
renaming, I documented one more difference among the three options (only
"cache" supports slow ACLs).

lookup_hit has not been implemented yet. Volunteers welcome, but it may
be a good idea to finish the "cache" removal discussion first because it
may affect how lookup_hit is implemented.

Yeah. None planned from me at this point, but I hope somebody will write
it once the discussion is over.
