refresh_pattern not working?

Running 3.1.12.2 here. I have the standard refresh_pattern in my config:

refresh_pattern -i (/cgi-bin/|\?) 0 0% 0
refresh_pattern . 0 20% 4320

yet URLs with '?' in them are still being cached. Example:
