Ok. Lets do this in two phases
1. Make our server side HTTP/1.1. This is the simpler of the two,
requiring only chunked decoding and a bit of header cleanup.
2. Client-side HTTP/1.1. Complicated a little by Expect & 100-continue
requirements. Not 100% sure how to fit that 100 response into the
response chain, but I suppose it would be acceptable to synthesize it as
I haven't seen any response headers used in the wild..
