Request for one more Squid error page HTML cleanup

We should add charset encodings to all error pages not using US-ASCII
by using the "standard" meta header. I see the addition of a doctype
as likely to prevent some browsers from "autodetecting" the charset.

Squid can later be extended to do the correct thing and parse the meta
header to include correct information in the HTTP headers, but this
is not critical for correct display and can wait for Squid-2.6.
