I don't quite know how ftp is handled in squid. I can only think
 of IIS timestamping files in way that squid thinks they are brandnew.
 If IIS does not support MDTM, is there easy way to workaround that?

I think FTP Cache doesn't work because FTP IIS 
Server does not support
MDTM.
