squid-HEAD bugs

hi all,

Even though I said I wouldn't be touching squid, I've found
some fun bugs in squid-HEAD.

We seem to be leaking request_t, aclCheck_t, short strings, and
httpHeaderEntry, rather reliably.

I noticed this whilst playing with modio, which is what I
was hacking on (I decided to merge it with squid-HEAD..)
