Proof-of-concept: dynamic starting and killing servers

Hi,
this patch is rough around the edges: buyer beware :-]

Specifically: the metrics for starting and killing servers are probably all
wrong.
I've only commented out a bunch of code: I'll clean that up once the
functionality it correct.
It's diffed against NTLM cvs code- you may need to apply by hand for
non-ntlm source trees.
