Squid log questions

Hi,

        My co-worker found that in Aug 1 sv log there were some URLs
containing "(null)". What does it mean?

        Another question: does Squid treat "http://foo.com" different from
"http://foo.com/" ? How about "http://foo.com/hello" and
"http://foo.com/hello/" ? Finally, how about "foo.com" and
"http://foo.com" ? (When I say different, I am saying Squid is going to
cache them in two copies despite the fact that they are the same.)
